The Blue Rivers Area Agency on Aging (BRAAA) Care Management
Program is designed to identify and coordinate community resources and services
to assist the frail elderly to remain at home. Care Management is a state-certified
program.
Care Managers conduct a comprehensive assessment of an individual
to determine their needs. The Care Manager then develops a care plan and implements
the plan by arranging the services needed, The Care Manager also provides adequate
follow-up with each individual to assure satisfaction and to identify changing needs.
The Care Management Program determines the individual’s ability
to share the cost of the program by applying their monthly income to a sliding fee
scale. Individuals will not be denied service base on inability to pay.
Care Management is available in all eight counties of our service
area. Referrals are accepted from any source.
